Search Aging & Disability Services in Wake County
The Center for Volunteer Caregiving is a private, nonprofit organization formed in 1992. Its mission is to engage the community in providing volunteer services to improve the lives of seniors, caregivers, and adults with disabilities.
The National Alliance on Mental Illness (NAMI) is the nation's largest grassroots mental health organization. NAMI Wake County is an affiliate of NAMI and NAMI NC. NAMI Wake County provides free education, support and advocacy for individuals affected by mental illness.
Senior CommUnity Care of North Carolina is the PACE (Program of All-Inclusive Care for the Elderly) program serving all of Wake and Durham county and parts of Granville county. A national model of healthcare with 121 locations in 32 states and 11 locations in North Carolina, SCCNC provides healthcare and social services to help people 55+ stay in their home.
The mission of First in Families of Wake County is to help people with disabilities and their families to believe in their dreams, achieve their goals and give back to others. We provide direct family support to meet the requests of individuals and families for needed goods and services to enhance their ability to live and thrive in the community. We also provide information and referral to other community services and supports. By stretching our public funding through donations and discounts from local businesses and sponsors we are able to provide more assistance to families in need.
Since 1979, Transitions LifeCare (founded as Hospice of Wake County) has provided compassionate end-of-life care to people of Wake and surrounding counties. Transitions LifeCare strives to promote and improve our community’s quality of living and dying through advance care planning, palliative (comfort) care, hospice care, home care, private duty and bereavement services. The philosophy of Transitions LifeCare is that every individual deserves the quality of life that he or she wants near life’s end.
Independent Living assists eligible individuals with significant disabilities in obtaining services that:
- Provide an alternative to institutionalization, when possible.
- Improve functioning in the family, home and community.
- Assist in preparing a person for a vocational rehabilitation program.
Services may be provided directly, purchased, or coordinated through other community agencies. If Independent Living is unable to meet a person’s needs, that individual may be referred to other agencies for services as appropriate.
The mission of Meals on Wheels of Wake County is:
- To provide nutritious meals to the frail, homebound senior adults and adults with disabilities in order to improve health, reduce isolation, and prevent inappropriate institutionalization
- To serve nutritious meals and provide social and educational opportunities to persons 60 and over in congregate nutrition sites to promote independent living
Resources for Seniors, Inc. is a nonprofit agency serving senior adults and adults with disabilities in Wake County by providing home- and community-based services and information. The goal of Resources for Seniors is to allow senior adults and adults with disabilities to remain as independent and healthy as possible. The agency provides information and support for decision-making and offers direct services such as home care, adult day care, senior center programming, home repair and accessibility modifications, and more.
Southeastern Healthcare offers services to help alleviate the strain of in-home and in-community care for older adults and adults with disabilities who want to remain in their own homes. Through Adult Day Health, Home Health, Psychosocial Rehabilitation and other therapeutic services, Southeastern Healthcare assists seniors and adults with disabilities with vital activities of daily living, health monitoring, and social interaction.
