Senior CommUnity Care of North Carolina is the PACE (Program of All-Inclusive Care for the Elderly) program serving all of Wake and Durham county and parts of Granville county. A national model of healthcare with 121 locations in 32 states and 11 locations in North Carolina, SCCNC provides healthcare and social services to help people 55+ stay in their home. At our adult day health center and medical clinic, our team of professionals coordinates and provides medical and specialty care, transportation to the center and to all doctors' appointments, full medication coverage, medical equipment, many therapies, respite, personal care, home care and much more.
