About Us

Since 1979, Transitions LifeCare (founded as Hospice of Wake County) has provided compassionate end-of-life care to people of Wake and surrounding counties.  Transitions LifeCare strives to promote and improve our community’s quality of living and dying through advance care planning, palliative (comfort) care, hospice care, home care, private duty and bereavement services.  The philosophy of Transitions LifeCare is that every individual deserves the quality of life that he or she wants near life’s end.

Transitions LifeCare provides expert medical care plus supportive care for family members and caregivers.  Care can be provided in all settings including individual homes, nursing homes, assisted living communities or hospitals.  If appropriate, care can also be provided in the William M. Dunlap Center for Caring, a 20 bed hospice facility on Trinity Road near the PNC arena.

Hospice care is appropriate for all ages and disease types, not just cancer, if the individual has a prognosis of 6 months or less.  Hospice care is paid for by Medicare, Medicaid or Private Insurance.

Palliative Care is a specialty consultative service that utilizes medical doctors and specially trained nurse practitioners to visit individuals in hospitals, nursing homes, assisted living communities or individual homes.  This service provides a comprehensive assessment and establishes goals of care which are based on patient and family values and priorities. Palliative care is appropriate for individuals with advanced illness, can be provided concurrently with curative treatments and does not require a terminal prognosis.  Palliative care offers assistance by helping to relieve symptoms such as pain, nausea, anxiety or fatigue, as well as by providing supportive services for family or caregiver.
